System Office New Hires
This form is to be used when an employee joins a department (new hire or transfer from another University department) for the initial network access needed. Complete the
SO Network User Account Access Request Form. Once this form has been submitted and the provisioning complete, requests for additional network access the user may need are to be submitted via e-mail to
